Case Manager – Contra Costa Adult and Youth Continuum of Services

Salary: $24.00 – $28.35 Hourly

Location: Contra Costa County, CA

Contra Costa Adult Continuum of Services (CCACS)

CCACS provides safe interim housing and supportive services to help homeless adults secure permanent housing. Our mission is grounded in human dignity and the belief that housing is a right supported through advocacy and collaboration.

Contra Costa Youth Continuum of Services (CCYCS)

CCYCS serves homeless and at-risk youth, ages 18–24, operating with a philosophy that young people can make positive choices when given guidance, support, and opportunity. We value their strength, diversity, and resilience.

Supportive Housing Program (SHP)

SHP and Rental Assistance Programs assist individuals living independently through case management, counseling, and housing retention services. Staff also engage with landlords to expand housing opportunities.

Position Overview

Under the supervision of a Program Manager, the Case Manager will serve clients based on program assignment (CCACS, CCYCS, or SHP). You will support individuals experiencing homelessness by helping them access housing, income, healthcare, and other essential services.

Essential Duties

• Manage a caseload and conduct client assessments (VI-SPDAT, needs assessments).
• Meet weekly with clients to develop and monitor service plans.
• Assist clients with securing identification and enrolling in public benefits (e.g., Medi-Cal, CalFresh, SSI).
• Connect clients with internal and external service providers.
• Provide direct support, including transportation using an agency vehicle.
• Maintain accurate documentation in both hard copy and the HMIS database.
• Educate clients on life skills, budgeting, and community reintegration.
• Facilitate family reunification and conflict resolution when appropriate.
• Report on client progress in case conferences and team meetings.
• Attend mandatory trainings, meetings, and case reviews.
• Other duties as assigned.

Program-Specific Responsibilities

CCACS – General Case Manager

• Perform all duties listed under "Essential Duties."

CCACS – Housing Case Manager

• Help clients create and follow housing plans.
• Conduct outreach to landlords and housing providers.
• Lead workshops (e.g., Ready-to-Rent, Financial Literacy).
• Support clients in attending housing appointments.

CCACS – Income Case Manager

• Assist clients in applying for income sources (e.g., GA, SSI, SSDI, employment).
• Develop individualized income plans.
• Refer clients to employment and education resources.
• Teach financial literacy individually and in group settings.

CCYCS – Youth Shelter Case Manager

• Work with homeless and runaway youth, ages 18–24.
• Provide trauma-informed support and strength-based case management.
• Maintain aftercare follow-ups (30, 60, 90 days) to ensure youth remain housed.
• Support Shelter Specialists with general milieu management when needed.

CCYCS – Housing Case Manager

• Conduct housing assessments and planning for youth.
• Coordinate with landlords and housing providers.
• Support youth in attending housing appointments.
• Collaborate with internal and external teams.

SHP – Housing Support Case Manager

• Build and maintain relationships with local landlords and agencies.
• Respond to community inquiries regarding housing.
• Evaluate resident housing for safety.
• Support tenants with landlord conflicts as needed.

Qualifications

Required:

• AA/AS degree in Public Health, Social Work, or related field.
• One (1) year of case management experience with homeless populations.
• (Or substitute with 3 years of experience plus HS diploma/GED)

Preferred:

• BA/BS in a related field.
• Bilingual in Spanish.
• Experience with street outreach and behavioral health services.
• Familiarity with Contra Costa County services and HMIS systems.
• Lived experience with homelessness and/or behavioral health services.

Skills and Competencies:

• Strong interpersonal, written, and verbal communication skills.
• Ability to work independently and as part of a team.
• Knowledge of Motivational Interviewing, Trauma-Informed Care, and Harm Reduction.
• Proficient in Microsoft Office and data entry.
• Must be non-judgmental and culturally competent.
• Valid CA driver’s license and clean DMV record.
• Must be 21+ and willing to participate in the DMV Pull Notice Program.
• Must pass background check and fingerprint clearance.

Work Environment & Physical Requirements

Environment:

• Office setting, field work, and homeless shelters.

Physical Demands:

• Frequently: Sit, stand, handle objects, and use a computer.
• Occasionally: Walk, reach, bend, lift/push up to 50 lbs.
• Constantly: Visual focus required.
